*1. Blood Test Report

The Blood Test Report needs to include results of HIV, Syphilis/V.D.R.L, HbsAg and HCV.

*2. Certificate of Non-criminal Record

It needs to be issued by the relevant department of the applicant’s country, and the validity period is six months. If it is not possible to provide it at the application stage, the Undertaking for a Clean Criminal Record can be signed as a temporary substitute document.

Please expatiate on the major or research field you would like to study at DUT, and this needs to be signed by the supervisor.

5. Acceptance Letter from DUT Supervisor (It is not obligatory)

*9. Undergraduate school transcript

Academic Transcripts in languages other than Chinese or English should be translated into Chinese or English and be certified by notarization. Both original and official English translated version are required.
Transcripts must be certified by the Chinese Embassy (Consulate) in your country or notarized by a notary office in China.

*10. Bachelor's degree diploma

Certificate in languages other than Chinese or English should be translated into Chinese or English and be certified by notarization. Both original and official English translated version are required.
Graduation certificate must be certified by the Chinese Embassy (Consulate) in your country or notarized by a notary office in China.

*11. Two letters of recommendation

Written in English from two respectable professors. including professors' contact no, email address and handwritten signature.

*12. Physical Examination Form(only valid for 6 months)

Please download the template, print it and go to do physical examination in hospital and ask the hospital to fill it according to your results, sign it and stamp it then send us the scan copy of the complete form; Medical examination results will be valid for 6 months; The medical exam MUST cover all the items listed in the Physical Examincation Form; Incomplete records or those without the signature of the attending physician, the official stamp of the hospital or a sealed photogragh of the applicants are invalid

Tips:
For applicants who are in China: If you are an employee, you still need to provide employment separation certificate and your current residence permit. If you are a student, you need to provide transfer certificate, leaving certificate, certificate of attendance rate issued by previous school and your current residence permit.
